What Data We Collect From You through a Form
By registering for The Medical City Enterprise Lifelong Learning Academy, the employees, allied health professionals, and nurses will be asked to provide the following personal and sensitive personal information to facilitate registration for the Enterprise Lifelong Learning Academy, information we collect may include but is not limited to:
· Name
· Email Address
· ID number
· Designation
· Department
On the other hand, medical trainees (residents and fellows) and consultants will be asked to provide the following personal and sensitive personal
information to facilitate registration for the Enterprise Lifelong Learning Academy, information we collect may include but is not limited to:
· Name
· Email Address
· Department
· PRC ID number
· ID number (for medical trainees only)
· School ID number (for interns and clerks only)
· Year level (for interns and clerks only)
We only process your information if we have your consent to do so, or where we are permitted by law.
How We Use Your Data
We use the information that we collect about you to effectively run the Enterprise Lifelong Learning Academy. We use your data to:
· Fulfill registration and account creation to the Enterprise Lifelong Learning Academy
· Enroll in specified courses required by respective department/section in compliance with training requirements.
· Monitor enrollment to courses of specified users in compliance with department/section requirements.
· Record activity logs and course test/s scores (if applicable).
How We Disclose/Share Your Information
We may share certain data about you to individuals and entities (i.e. cloud hosting provider) who assist us in fulfilling our responsibilities as your Program Owners or when required to do so by applicable law. The entity to whom your data may be requested from is Nephila Web Technology, Inc. (support@nephilaweb.com.ph). We only share your information to the Learning Management System (LMS) hosting provider who have agreed to ensure an adequate level of data protection to your personal data that is consistent with the Data Privacy Act of 2012. We may share data in other ways such as when it is aggregated or de-identified or when we have your consent. On the other hand, assigned staff of The Medical City as site administrators, course managers, course creators, and teachers have different degrees of access to your data including but not limited to the following:
· Activity logs
· Course progress
· Test score (if applicable)
Data Retention
The Medical City Enterprise Lifelong Learning Academy shall store and keep secure provided information while it is needed for the abovementioned purposes. Thereafter, they will be disposed of securely and permanently.
Your Data Privacy Rights
We have procedures for you to exercise your rights such as but not limited to: being informed, capacity to object, access, rectify, erase or block, transfer data, file a complaint and demand payment for damages. All inquiries and requests in upholding your rights must be made in writing. The Medical City Enterprise Lifelong Learning Academy reserves the right to require further documentation, depending on the nature of your inquiry/request.
